Youth Basketball Camp Application

Monument Valley High School – Kayenta, Arizona

(For boys and girls who will be in grades 2 – 8 in the Fall of 2022)

$40 per player

Camp to be conducted by the MVHS Girls’ Basketball Club Players and Coaches

GENERAL INFORMATION

• ALL PARTICIPANTS ARE REQUIRED TO WEAR A MASK FOR THE DURATION OF CAMP
• Registration for the camp will take place at the first session. Players must bring a completed registration form and $40 camp participation fee. **Checks or money order can be made out to MVHS Girls’ Basketball Club***
• Camp T-shirts will be given upon payment of registration fee.
• Awards will be presented to qualifying campers at the end of the last day of camp.
• Campers will be involved in strenuous physical activity. It is advisable for their safety that they report to camp in good physical condition.
• Campers will participate in a wide variety of basketball skills, drills, fundamentals and competitive games.
• Players must supply their own shoes and workout attire.
• Parents are responsible for their child’s transportation to and from camp.
• Monument Valley High School, Kayenta Unified School District #27, and the camp staff are not responsible for any injury, illness, and/or loss to personal belongings that may occur during the 2022 MVHS Youth Basketball Camp. On the first day of camp, participants will be divided into groups based on age/grade and will be instructed based on their appropriate level of development.
